Registered Nurse (PRN)🩺

Position Overview

WrightChoice Healthcare Services is seeking a compassionate, licensed Registered Nurse (RN) to join our private home care team on a PRN basis. As an RN with WrightChoice, you will play a critical role in ensuring that clients receive high-quality, person-centered care in accordance with their individualized service plans.

This position complies with the standards outlined in Georgia Department of Community Health Rule 111-8-65, ensuring clinical oversight, client assessments, and supervisory visits are performed to the highest standard.

Key Responsibilities
  • Conduct initial in-home client assessments to develop appropriate service plans.

  • Provide ongoing supervision of unlicensed staff such as Personal Care Assistants (PCAs) and Certified Nursing Assistants (CNAs).

  • Perform reassessments at required intervals or as client conditions change.

  • Maintain accurate clinical documentation in compliance with state regulations and company policies.

  • Ensure all services align with the client's physician orders and service plan.

  • Respond to care concerns or changes in condition with professional judgement and follow-up.

  • Educate clients and families on health management and safety protocols.

  • Participate in quality assurance initiatives and continuous care improvement.

Qualifications

  • Active and unrestricted Registered Nurse license in the State of Georgia.

  • Minimum one (1) year of experience in a home care, hospice, or clinical setting preferred.

  • Knowledge of and compliance with Georgia Rule 111-8-65 governing private home care.

  • CPR Certification (American Heart Association or Red Cross).

  • Valid driver’s license and reliable transportation.

  • Strong communication, organization, and documentation skills.

  • Ability to work independently and exercise sound clinical judgment.

Personal Care Assistant (PCA)🧑‍⚕️

Position Overview

WrightChoice Healthcare Services is seeking dedicated and compassionate Personal Care Assistants (PCAs) to provide in-home support for clients who require assistance with daily living activities. As a PCA, you will help clients maintain their independence, dignity, and comfort—all within the safety of their own home.

This position follows the guidelines of Georgia Department of Community Health Rule 111-8-65, ensuring safe, competent, and supervised personal care.

Key Responsibilities
  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, toileting, grooming, and oral hygiene.

  • Support mobility and transferring (e.g., from bed to chair).

  • Perform light housekeeping duties: sweeping, vacuuming, laundry, dishes, and bed making.

  • Prepare or assist with meal planning and preparation.

  • Provide medication reminders (no medication administration).

  • Offer companionship, monitor client safety, and report any changes in condition.

  • Follow individualized service plans developed by the RN supervisor.

  • Maintain accurate documentation and communicate with the care team as needed.

Qualifications
  • Must be at least 18 years of age.

  • High school diploma or equivalent preferred.

  • Must meet one of the following:

    • Completion of a PCA training program including 40 hours of curriculum and skills assessment.

    • Current certification as a Certified Nursing Assistant (CNA) in Georgia.

    • Successful completion of the NLN PCA competency exam and skills assessment.

  • Ability to read, write, and follow instructions and care plans.

  • Ability to pass a background check, TB test, and meet all compliance requirements under GA Rule 111-8-65.

  • CPR and First Aid certification preferred.

  • Reliable transportation.

Position Overview

WrightChoice Healthcare Services is seeking compassionate and dependable Companion/Sitters to provide non-medical in-home support for clients who need social interaction, supervision, and basic assistance. This role is vital in helping individuals maintain their independence and quality of life in a safe, familiar environment.

As required under GA Rule 111-8-65, Companion/Sitters must be competent, trained, and supervised to ensure the safety and well-being of our clients.

Key Responsibilities
  • Provide companionship and social interaction through conversation, reading, games, or walks.

  • Assist with light housekeeping, including tidying, laundry, and dishes.

  • Support meal preparation and encourage proper nutrition.

  • Offer medication reminders (no administration).

  • Supervise client safety and report any changes in condition or behavior to the care team.

  • Accompany clients to medical appointments or errands as needed.

  • Follow individualized service plans and agency protocols.

Qualifications
  • Must be at least 18 years of age.

  • Must be able to read, write, and follow written instructions.

  • Must meet one of the following:

  • Completion of a Companion/Sitter training or orientation provided by the agency, OR

  • Successful passing of a competency assessment related to duties and safety procedures.

  • Demonstrated understanding of infection control, emergency response, and home safety.

  • Patience, empathy, and a sincere desire to help others.

  • Reliable transportation.

  • Background check and TB test required.

  • CPR and First Aid certification preferred.
