What Is the WrightChoice Community Care Plan?

The WrightChoice Community Care Plan is an income-based, reduced-cost program for seniors and adults with disabilities in Georgia who need in-home support but don’t qualify for Medicaid or VA benefits. We believe everyone deserves compassionate, quality care — regardless of income.

How Pricing Works

Your care cost is determined by your income and household size using the Federal Poverty Level (FPL) as a guide.

Community Floor Rate. WrightChoice will not set a Community Care Plan rate below the current Georgia Medicaid T1019 effective hourly rate (4 × the per-unit rate), rounded up to the nearest $0.25. If a client’s sliding-scale calculation falls below this floor, the floor rate applies. We update the floor within 10 business days of any Medicaid rate change.

  • Respite care for family caregivers

How to Qualify

To enroll, you must:

  • Reside in one of our Georgia service counties

  • Provide proof of income (tax return, SSI/SSD, pay stub)

  • Complete a short needs assessment with our care team

  • Not currently receiving Medicaid or VA home care services

Step 2: Submit Your Info
We’ll ask for income verification, ID, and a signed consent form.

Step 3: Get Matched with a Caregiver
We’ll create a personalized care plan based on your needs and assign a compassionate caregiver who fits your schedule.

We use your tax household (people on the same federal tax return) and gross income to set your sliding-scale rate. Social Security retirement and SSDI count; SSI and non-cash benefits do not. If someone you live with does not support you and doesn’t claim you, we can review hardship and may price using your income only. Your rate will never be below the current Georgia Medicaid T1019 hourly “floor.”
